Daily Physical Activity Consent Form
As part of our learning in Physical and Health Education, students at West Bench will be participating in various exercise activities on and off school grounds within the West Bench Community. Activities focus on developing endurance, strength, flexibility, and may take a different form depending on the weather. Students will participate in activities such as School-Wide Walks/Runs, Walks to Selby Park, and neighborhood hikes.
School-wide activities take place at least three times a week throughout the year, weather permitting, and off-site activities require permission in order for your child to participate. It is imperative that ALL students come prepared for physical activities EVERY day. Please ensure your child has properly fitting running shoes and comfortable, season appropriate, clothes that they can move freely in. For indoor activities, clean-soled non-marking shoes are important to protect the gym floor. Older students may be asked to change into gym strip.
Accidents can be the result of the nature of the activity and can occur with or without any fault on either part of the student, or the school board or its employees or agents, or the facility where the activity is taking place. By allowing your child to participate in this activity, you are accepting the risk of an accident occurring, and agree that this activity, as described above, is suitable for your child.
